National Health Check-up Scheme for Workers

Context
The Union Ministry of Labour has announced a nationwide health check-up programme for labourers aged 40 years and above that is for free and will be implemented starting May 2026. This is being undertaken under the Employees’ State Insurance Corporation (ESIC) and is part of a new set of Labour Codes to deliver preventative care and early detection of disease to millions.
Key Features of the Scheme
- Target Demographic: All insured workers aged 40 years and above under the Employees’ State Insurance Corporation (ESIC) framework.
- Preventive Focus: Early detection of non-communicable diseases (NCDs) such as cardiovascular disease, diabetes, hypertension.
- Implementation Network: The large network of ESIC Medical College & Hospitals & associated hospitals are involved in the implementation across the country through annual screenings.
- Mandatory Screenings: Workers who work in hazardous jobs involving toxic chemicals, toxic materials, or heavy machinery require medical screenings, whether young or old.
- Comprehensive Care: In addition to check-ups, the programme includes systematic health record keeping and delivery of required interventions and medicines identified at the check-ups.
Why focus on workers over 40 years of age?
The initiative focuses on the 40+ age group as the risk of chronic and sedentary lifestyle related diseases becomes high after 40 years of age. Regular screenings serve as an effective preventive measure for India’s aging population, offering early detection of potential future problems and thus the opportunity to make lifestyle changes or interventions before they become serious.
Significance of the Scheme
- Preventive Healthcare Focus: Transitions from curative to preventive care and early detection, early management of non-communicable diseases (NCDs).
- Implementation of New Labour Codes: Complies with requirements of the Occupational Safety, Health and Working Conditions (OSH) Code 2020.
- Workforce Productivity: A healthier workforce contributes to increased productivity and decreased early morbidity among incumbent workers.
- Social Security Expansion: Promotes occupational health systems, especially for hazardous industries.
Role of ESIC and Infrastructure
The programme is being implemented through the vast network of ESIC hospitals and dispensaries and is backed by the government, which promises that any diseases are treated free of charge if detected. This lessens the shock of unplanned medical costs on the working class.
Challenges and Implementation
- Coverage Gap: There are 94 crore workers and only 31 crore workers are on e-Shram portal and integration to ESIC is still underway.
- Infrastructure Constraints: ESI funds are sizable, but the number of beds and doctors still needs to be enhanced, and sometimes provided through PMJAY empanelled facilities.
Way Forward
This is an important measure to ensure social security and dignity to India’s working population. Emphasis will be placed on ensuring successful integration of the unorganized sector employees and augmenting infrastructure of ESIC for making the workforce fully covered, healthy and productive.
